Client Agreement & Activity Consent
1. Owner and dog details
2. Health and behaviour disclosure
Dogs joining the pack must be non-aggressive toward dogs and people, have up-to-date vaccinations including rabies and DHPP (or an equivalent titer test), and be spayed or neutered. Please attach a current vaccination record.
Recommended, not required: leptospirosis and kennel cough (bordetella). Adventures take place outdoors and in a group setting, where exposure to both is more likely. Scooby’s Den does not require these vaccinations and does not give veterinary advice. Please consult your veterinarian before deciding.
I confirm my dog meets these requirements.
Disclose previous incidents even if you believe they are resolved. Scooby’s Den will review suitability before accepting your dog. Inform us of illness, injury or changes before an outing.
Activities & Permissions
3. Core adventure activities
Activities may include supervised group walks, hikes, sniffing, enrichment, outdoor play and rest breaks. Routes, activities and duration may change for weather, trail conditions and each dog’s needs. A meet-and-greet or trial day may be requested before pack participation.

4. Optional permissions — mark one choice per line
Vehicle transport: Pickup, drop-off and transport between agreed adventure locations, with appropriate restraints.

Swimming: All swimming activities are supervised and life jackets are provided for every dog. Swimming will only be offered when conditions and the dog’s suitability allow.

Off-leash activity: Only with this specific consent, after suitability assessment, and where legally permitted and covered by Scooby’s Den’s insurance. Consent does not guarantee off-leash participation.

Home access while I am absent: I authorize agreed entry solely for my dog’s pickup and return. Access arrangements must be agreed separately; do not write access codes on this form or in a payment reference.

An unanswered optional permission is treated as “No.” Changes must be communicated and acknowledged before the next outing.
5. Photos and videos — optional
We aim to send individual updates when possible. Please choose separately whether photos and videos of your dog may be used on Scooby’s Den’s website and social media. Declining public use does not affect care or eligibility.
I allow public use of my dog’s photos/videos. I do not allow public use.
Owner contact details and home access information will not be published. You may withdraw permission for future use by contacting us.
Emergency Care & Booking Terms
6. Emergency veterinary authorization
If my dog becomes ill or injured, Scooby’s Den will contact me immediately and try my emergency contact if I cannot be reached. I authorize transport to the nearest emergency veterinary hospital when needed.
If neither contact can be reached, I authorize a veterinarian to provide urgent assessment and stabilization that cannot safely wait, up to the amount I enter below. Scooby’s Den must seek further authorization where possible. Nothing here authorizes euthanasia.
I agree to pay veterinary charges authorized under this agreement, subject to applicable law and any responsibility Scooby’s Den may have for an incident. This authorization does not waive a claim or determine legal responsibility for costs.
7. Payment and confirmation
Request your service and dates first. Scooby’s Den confirms availability and the total, reviews the signed agreement and required records, and then requests payment. A booking is confirmed once these steps are complete and payment is received.
Send Interac e-Transfer to scoobysden26@gmail.com, using Dog’s Name + Package as the reference. Example: “Lola – Full Day Pickup/Drop.”
Five-day packs are prepaid, valid for 60 days, subject to availability and intended for one household. The service and total will be confirmed in writing before payment.
8. Changes, cancellations and weather
Bookings changed or cancelled at least 24 hours in advance can be moved to another available date. Cancellations within 24 hours are non-refundable and use the booked package day, subject to applicable consumer rights.
Normal rain and snow may be part of an adventure. Unsafe conditions may require service to be delayed, modified or cancelled. If Scooby’s Den cancels for safety, you receive full service credit and do not lose a package day. This does not limit any refund or other rights required by law.
Morning pickups start at 8:30 AM. Times are estimates. Confirmed clients receive an optional WhatsApp invitation for route status and ETA updates. Do not share addresses or access codes there. WhatsApp may display your name or phone number to other participants depending on group settings; joining is optional.
Meet-up service: We wait up to 10 minutes at the agreed time/location, then continue the route. Contact us to arrange a safe handover if delayed. Additional care costs CAD $5/hour; alternatively, an agreed pickup or drop-off costs CAD $10 once, subject to availability. We confirm any charge with you.
Two-hour walk: Time includes pickup, travel and drop-off. We aim for a suitable trail within 5 km; otherwise, a two-hour leashed pack walk with compatible dogs, with the plan agreed beforehand.
Risk Acknowledgement & Signatures
9. Understanding adventure risks
I understand that outdoor activities, dog interactions, swimming and vehicle travel involve risks even with reasonable care. These can include slips, cuts, bites, illness, parasites, escape, traffic incidents, water-related injury, serious injury or death.
Scooby’s Den will exercise reasonable care, supervise activities and adapt plans to conditions and the dogs involved. We attach individual trackers, carry first aid, provide water breaks, check temperatures and watch for discomfort. Trackers and life jackets do not replace supervision. We try to clean dogs before returning them, but outdoor adventures can leave them dirty. No particular behavioural result or risk-free outcome is promised. I have had the opportunity to ask questions about activities and discuss my dog’s needs.
This is an acknowledgement of risks and consent to agreed services. It does not release Scooby’s Den from liability for negligence, waive statutory or consumer rights, or exclude any liability that cannot lawfully be excluded.
10. Information and ongoing consent
The information I provide is accurate to the best of my knowledge. I am the owner or have authority to authorize care. Scooby’s Den may use the information for arranging services, safe handling and emergency care, and share relevant details with a treating veterinarian when needed.
This agreement applies to future bookings for this dog until replaced or ended in writing. Material changes to terms require renewed agreement. I will promptly update contact, health and behaviour details. Optional permissions can be changed in writing before an outing.
I have read all four pages, completed my permissions and emergency limit, and agree to the services and terms described. I will keep a copy of the signed agreement.
